I understand that as a UIC Shareholder I may be eligible for UIC loyalty benefits. I will be eligible for such benefits only after UIC Stock Department verifies my shareholder status.

I understand that in order to receive any UIC Shareholder benefits I am required to notify the cashier that I am a UIC Shareholder and present a UIC Shareholder or Government Issued ID at time of purchase.

I understand that I will earn one point per dollar spent at Stuaqpak. I also understand that my points cannot be redeemed until I reach a balance of 1,500 or greater.

I understand that my points balance will expire if I have not accrued points during any 12 month period.

I understand that I receive 1 mile for every 3 points earned and redeemed. I understand that miles are funded once a quarter and take up to 90 days to show in my account. I also understand that I must have a minimum of 1,500 points to convert to miles at the time of funding.

I understand that I receive $1 in store credit for every 150 points earned and redeemed. I understand that store credit is funded once a quarter and takes up to 30 days to receive a store credit. I also understand that I must have a minimum of 1,500 points to convert to store credit at the time of funding.
